Writing out your prayers can be a great habit to form. 
I find I sometimes need a focus so that I'm not just rattling off my list of wants and needs to God, so I've put together a themed prayer guide.
 
This prayer guide is themed around Loved Ones. Each day has a theme and a corresponding scripture that you can add in to your prayers, and write them down.
 
There are 2 plans on one A4 sheet, so you could either share one with a friend, or I've laminated one to keep stashed in the back pocket of my journal to use all the time, then I've used the second copy to cut up and collage onto some of my prayer pages.
